The Department of Veterans’ Affairs provides physiotherapy services for eligible DVA recipients. If you are a current or former serving member of the Australian Defence Force, you may be an eligible recipient of DVA allied health services. These benefits may also be available to a spouse, widow or child of a serving member of the ADF.
To be assessed for eligibility of these benefits, an approval from a General Practitioner on a current D904 referral form is required. As part of the DVA Benefits Schedule, serving members of the ADF will be able to access physiotherapy services in conjunction with the type of card they have received. As part of your treatment as an eligible DVA recipient, Steer Physiotherapy will liaise with your GP to determine a suitable long term management plan that meets your medical needs.
Prior to attending your appointment at Steer, we will require some additional information which includes:
- DVA number/card
- GP referral
- D904 form completed within the last 12 months
- Any relevant scans/imaging results
